Collecting Information for an Access Database by E-mail

Select the fields you wish to collect

In the next wizard screen (shown above), you'll see a listing of available fields. Use the arrow buttons to move the fields you wish to collect from the left side of the window to the right side.

If you wish to use the existing field name in the form, you do not need to make any changes. However, if you wish to use a user-friendly name, you may provide it in the "Label to display in front of the field in the e-mail message" textbox.

After adding all relevant fields, click the Next button to continue.
